We are so grateful that you chose our practice to care for all of your vascular needs. Please read our welcome letter so you know what to expect in your care.

Medical Records Release
Per HIPAA guidelines, in order for us to receive or release records to and from our practice, we must have a records release form on file. Without this form, we cannot release your medical records.

The average turn around time for records release is 72 hours. We thank you for your patience.

New Patient Paperwork
Prior to your first visit with us, we ask that all new patients fill out this packet so we can get to know you and tailor care to you.

We ask that alongside this paperwork, you bring a list all of all your current medications (vitamins and over-the-counter included) and a valid ID and insurance card to your first appointment.
